Age UK comments on discharge from hospital research

In response to new research by Brighton & Sussex and King's College London, showing that 37% of older people suffer from medication harms on discharge from hospital,
Caroline Abrahams, Charity Director at Age UK said:
“Modern medicines bring huge benefits and are essential in helping people to stay well, manage their health conditions and recover from serious illness. For many people they are lifesaving. However, today’s report is also a shocking and timely reminder that medication needs to be carefully managed, especially when someone is taking several different ones or is prescribed something new. It is vital that when anyone is discharged from hospital they are given the information they need to understand how to carry on taking their medications at home, and any changes are quickly and accurately communicated to their GP or carers.”
“It is important that people continue to take their medications as prescribed, however, if you have any concerns consult your GP or pharmacist as soon as possible.”
